Appointment & Check-In Process

Appointments can be scheduled in person or by phone. Patients receive a packet via email or in person, which can be returned at check-in or emailed beforehand. Our team confirms appointments a day prior. Check-in is completed at the front desk, and patients receive a printed schedule of future appointments at checkout.

At Southwest Cancer Care, our patient intake process is designed to deliver exceptional efficiency and precision. Our New Patient Coordinators ensure that every interaction—whether inbound or outbound—prioritizes patient needs and medical accuracy.

Through proactive communication via calls, voicemails, or SMS, we ensure patients are well-informed and adequately prepared, creating a seamless transition into their care journey with us.

What to Bring to Your First Appointment

To help us provide the best possible care we’ll need information about your medical history and any prior treatments, please bring the following items to your initial visit:

We’ll need access to records related to your cancer diagnosis and any previous treatments. With your permission, we can request these directly. Records we may need include:

  • Notes from your physician
  • Imaging studies (such as X-rays, CT scans, PET scans, or MRIs) along with the official reports
  • Pathology, lab results, and surgical reports
  • Treatment summaries from previous providers

To coordinate your care, your oncologist will work closely with your referring doctor and/or primary care provider. This ensures everyone involved in your treatment is on the same page.

 

We may share test results and updates with your other physicians to support a collaborative approach to your treatment.

Please bring your current insurance card. If your insurance plan requires a referral for specialist care, confirm that your referring provider has sent it to us ahead of your appointment.

 

We’ll verify your insurance benefits and let you know if there are any out-of-pocket costs.

 

We’ll handle submitting your insurance claims.

 

Our financial counselors are available to assist with billing questions or concerns.

 

Please note: If we’re unable to confirm your insurance or referral before your appointment, your visit may need to be rescheduled or you may be responsible for the full cost.

We encourage you to bring a list of any questions or concerns you may have. Your doctor will take the time to explain your diagnosis, review treatment options, and outline the next steps in your care.

Please bring a current list of all medications you take including prescription drugs, over-the-counter medications, supplements, and vitamins.

 

You can either bring the original containers or a completed Medication and Allergy List form.
